Vision Insurance Claims
Vision insurance is a wellness benefit. It is for routine eye care, eyewear, and vision-related services, sometimes fully or partially covered or sometimes at a reduced cost. Medical insurance is for eye injury and/or disease; it may or may not require a copayment and/or deductible. We accept virtually all plans, and the doctor bills the correct plan depending upon the reason for the particular visit. It is your responsibility to provide your correct insurance information. Check with your HR Department if you aren’t sure. We file insurance claims electronically so that your claim gets to the insurance company as fast as possible. Within 30 days of the receipt of your claim, your insurance is legally obligated to pay each claim. After the 30 days, you are personally responsible for any balance left on your account. This applies whether your insurance has paid or not.
Please be aware that we file claims as a courtesy to our patients, but we cannot control, and are therefore not responsible for, how your insurance company handles your claims and what benefits they are willing to pay. We cannot guarantee what an insurance company will do with each claim. We are only able to provide an estimate of what your cost for your treatment will be.
Determining Vision Insurance Benefits
Be sure you are familiar with your insurance benefits so that you will be prepared to pay the amount that your insurance does not cover. Our office does not determine your eye care benefits.
Many patients think their insurance pays 90-100%, when in actuality their coverage may be much less. Insurance coverage can often range between 50-80% of the total cost. The percentage your insurance company will pay depends on the type of policy you or your employer has set up with that particular insurance company. We recommend that you contact your insurance company to verify your benefits before scheduling treatment.
Fees and Insurance Reimbursements
The insurance company sets its own schedule and they have their own set of fees that are usually different from other companies. The fees can vary greatly. After collecting all the information about a claim, the insurance company will choose a reimbursement level that they feel is acceptable to their own standards. The data that they use can be a few years older than the claim date, which gives the opportunity for the insurance company to collect a profit.
Sometimes an insurance company will reimburse its client at a lower rate than the actual fee of our office. In this instance, a company will say that the reimbursement was reduced due to the office fee having exceeded the usual, customary, or reasonable fee (UCR) used by the insurance company. Don’t let this give you the idea that our service fee is any higher than another office. This is misleading and inaccurate.
Unfortunately, insurance companies will tell you that the doctor is charging you too much instead of admitting that the company is “underpaying” or that your benefits are too low. A lower UCR figure will often be used for a less expensive insurance policy.
Deductibles and Copays
You also need to be aware of your deductibles and your copayments. We will estimate the benefit to be paid. The deductible is considered first and paid by the patient. This amount is then subtracted from the balance owed.
REMEMBER TO KEEP US UPDATED ON ANY CHANGES WITH YOUR INSURANCE, INCLUDING POLICY NAME, INSURANCE COMPANY ADDRESS, OR CHANGES IN EMPLOYMENT.
